“With only 19 percent of employees using their company’s Wellness Program, it is evident that organizations need to change how they promote their Wellness Programs. These programs may contain all of the ingredients for success – health education, counselors and resources – but the “recipe” needs to be customized.”

Centurion’s Wellness model is designed to address individual client needs instead of using a “one size fits all” approach. Our process includes:
  • Assessment: We examine your organization’s profile and goals to build a program that leverages available services with the results of our health risk assessment (HRA) and biometric screenings in order to provide a solid baseline of information to formulate immediate and future programs.
  • Customized Intervention Program: Participants in our Customized Intervention Program receive highly engaging personalized coaching sessions throughout the year, addressing their specific clinical and lifestyle related risk factors. Each participant’s program and goals focus on the combination of his or her total physical, mental, and emotional risks. Using our certified health coaches we are able to achieve high enrollment levels and maintain participation throughout the year.
  • Disease Management: Our Disease Management Program is geared to the management of many of the most costly chronic disease states, including:
    • Asthma
    • Coronary Artery Disease (CAD), Including hypertension and high cholesterol
    • Chronic Obstructive Pulmonary Disease (COPD)
    • Congestive Heart Failure (CHF)
    • Diabetes
  • Finally, our powerful analytics measure the impact of programs and interventions on your organization’s total healthcare cost so that you can easily measure program impact and make adjustments as needed.

The professional team at Centurion developed an overall strategy to creatively transition from a traditional fully insured health model to a more cost effective model based on rigorous analytics and algorithms that uncovered pathways to cost reductions. This was a big breakthrough for a very traditional culture, and it produced dollars to the bottom line within a very short time. Senior management was very impressed."
